Basic insurance pays no housework. Supplementary insurance pays CHF 10 to CHF 200 a day depending on the product, often CHF 30 to CHF 100, usually with a medical prescription and for a few weeks a year. The documents of the 12 insurers we read say nothing explicit about a person you employ yourself. That is why one question to your insurer is worth asking before the help starts.
What basic insurance pays, and what it does not
Basic insurance pays for care, not housework. The list of care services in KLV Art. 7 includes neither cleaning nor cooking, laundry or shopping. This also applies when the Spitex comes: the care goes through the insurer, the housekeeping you pay yourself.
What it pays
- Midwife after the birth, at home
- Care by the Spitex, with a medical prescription
- Acute and transitional care after hospital, at most 2 weeks
What it does not pay
- Cleaning, laundry, cooking, shopping
- Looking after children
- Spitex housekeeping
After the birth: the midwife, not the household
In the 56 days after the birth, basic insurance pays for up to 16 midwife home visits after a first birth, a premature birth, multiples or a caesarean, otherwise up to 10. In the first 10 days, up to 5 second visits on the same day are added, more only with a medical order. Deductible and co-payment do not apply during this time.
The midwife is there for mother and child. Laundry, shopping and older children stay with you. For the household in the postnatal period, the only options are supplementary insurance, family or paid help.
The legal basis
Law: KVG Art. 25 and KLV Art. 7 govern care, KVG Art. 25a acute and transitional care, KVG Art. 29 and KLV Art. 16 maternity, KVG Art. 64 para. 7 the waiver of cost sharing.
Supplementary insurance: this is a private contract under the VVG. Only the conditions of your product count; there is no statutory right to household help.

After a birth: medically prescribed, staff recognised by the insurer, relatives only with lost earnings.
Who may help: Professionally on own account or for an organisation
For this situation we found no contribution in the documents we read from Assura, SWICA, Visana. That does not mean it exists nowhere.
Does the insurer also pay if you employ someone yourself?
No table can tell you for sure: none of the documents we read mentions an employment contract. The wording does fall into five groups, though, and they show you how open the question is at your insurer.
A private person too
CONCORDIA DIVERSA · CONCORDIA Spitalversicherung · CSS Ambulantversicherung myFlex · Sanitas Family · Sanitas Hospital Extra Liberty, Top Liberty · Sanitas Hospital Standard Liberty · SWICA Hospita · Visana Ambulant II, III
Wording: «von einer nicht zu den nächsten Angehörigen der versicherten Person gehörenden Person»Our reading: The wording does not exclude an employed person. But some insurers require an invoice with a calendar, and an employee does not issue invoices, she receives a payslip. The question below settles whether that is enough.
Professionally on own account or for an organisation
Atupri Mivita · Groupe Mutuel Global smart · Groupe Mutuel Optimum · Groupe Mutuel Premium · Helsana Hospital Eco · Helsana Hospital Halbprivat, Privat · ÖKK ELTERN, START · Sympany hospita
Wording: «beruflich auf eigene Rechnung oder für eine Organisation»Our reading: This describes self-employed people and companies. A directly employed household helper is neither, so the wording tends to go against her. That does not settle it: ask beforehand.
An assistance service
Assura Complementa Extra
Wording: «les frais pour la tenue du ménage par un service d'assistance»Our reading: By the wording, a service counts. A private person probably does not, not even an employed one.
Nothing on it
Helsana Hospital Flex · KPT Krankenpflege-Comfort · KPT Spitalkosten Flex Top · SWICA Completa Top, Completa Forte
Wording: «für Haushalthilfe auf Basis einer detaillierten Rechnung mit Kalendarium»Our reading: Shown costs or an invoice are required, nothing more is stated. Here the question is completely open.
No proof of cost up to an amount
Atupri Haushalt-Taggeld
Wording: «verzichtet Atupri auf einen Kostennachweis»Our reading: Up to CHF 50 a day the insurer asks for no proof of cost, so the daily allowance does not depend on who helps. Above CHF 50 tighter rules apply.
What applies to you if you employ someone
Whether the insurer pays or not: anyone who employs a household helper directly is an employer. In a private household, as a rule, every wage is subject to AHV from the first franc, and she needs accident insurance even for a few hours.
From the start, record what an insurer will want to see: employment contract, hours with dates, payslips. Sanitas and KPT, for example, require a calendar, meaning proof day by day.
The one question for your insurer
Send it in writing, before the first day of work, and keep the reply. Fill in only the brackets.
I have the supplementary insurance [product] with you: do you contribute to household help after [birth, hospital stay or illness], how much per day and for how long, do I need a medical prescription, and does this also apply to a person I employ directly, with payslip and hours list as proof?
And in this order
- Get the prescription, often the first condition.
- Send the question and wait for the reply. Some insurers require a request in advance.
- The help starts, with a contract and registration if you employ someone yourself.
- Submit the proof: invoices with a calendar, or payslip and hours list.
Who else pays
The insurer usually pays for only a few weeks. For longer, or without supplementary insurance, there are other bodies. Which of them applies to you depends on age, income and canton.
Supplementary benefits
If you receive supplementary benefits (EL), help at home is reimbursed as an illness cost, within a frame of at least CHF 25'000 a year for single people and CHF 50'000 for married couples. For private help, each canton sets its own limits: Basel-Stadt pays at most 16 hours a month at CHF 30, Aargau at most CHF 25 an hour and CHF 4'800 a year.
Helplessness allowance
Anyone who permanently needs help with dressing, personal care or eating can apply for a helplessness allowance. With the AHV it takes 6 months of uninterrupted helplessness, then at home you receive CHF 252, CHF 630 or CHF 1'008 a month, without receipts.
IV assistance contribution
If you receive an IV helplessness allowance and live at home, you can get an assistance contribution. It also counts help in the household, but only for a person you employ under contract, and not for direct-line relatives or your partner.
Accident insurance
After an accident, the accident insurer contributes to non-medical help at home, as far as the helplessness allowance does not cover it (UVV Art. 18). The ordinance does not say how much. Ask.
Red Cross childcare
For children up to 12 when a parent is out of action, in Zurich also for exhaustion after a birth. SRK Zurich charges CHF 6 to CHF 30 an hour by tax values, SRK Bern CHF 7.50 to CHF 40 by monthly wage.
Family help and the commune
Some regions have a family help service with rates by income. In the canton of Zug it costs CHF 12 to CHF 46 an hour, also during illness and after a birth. Your commune or social service knows what exists where you live.
Housekeeping from Spitex and Pro Senectute
You pay these hours yourself. Spitex Zurich charges CHF 31 to CHF 44 depending on income, Pro Senectute Aargau CHF 35 plus CHF 6 per visit. The comparison with Spitex shows how that looks against an employed person.
